Diabetes Data Dock 

The Diabetes Data Dock technology encompasses data integration capabilities (like EHR and wearables), a series of machine learning predictive models and a population health management platform with an associated reporting functionality. 

The D Data Dock can be used by operational and clinical teams to proactively engage high-risk individuals and offer them and their families a tailored intervention to improve glycemic control and reduce the likelihood of DKA hospitalization.


 Diabetes Technology Maturity Model (DTMM)

The DTMM is a structured framework to help healthcare institutions assess, guide and advance their integration of diabetes-related technologies.

The evaluation tool enables institutions to: 

  • Assess their capabilities, identify gaps and advance systematically across key domains 
  • Align their processes, technology and outcomes to deliver more effective, data-driven and patient-centered care. 
  • Plan future technology-related investments and workflows according to a strategic framework. 
  • Promote alignment and ensure efforts lead to improved outcomes, increased efficiency and equitable care. 

 Rapid Learning Lab

The Rapid Learning Lab combines data and implementation science with quality improvement and change management. The purpose is to rapidly test interventions against a given outcome. 

Adoption of the RLL requires the sharing of:  

  • The design and outcomes achieved for validated and investigational interventions 
  • The operational and other feasibility constraints associated with each intervention  

A commitment to knowledge-sharing supports the development of the intervention library, which can be used by any member of the RTA to support data-driven care models for diabetic patients in their community.   


 
iCoDE

The Integration of Continuous Glucose Monitoring Data into the Electronic Health Record (iCoDE) facilitates the efficient upload and integration of continuous glucose monitor (CGM) data into the Electronic Health Record (EHR). The project aims to develop technical integration specifications and data integration facilitation guidelines.  

Over 130 individuals from more than 60 academic, government and clinical organizations, as well as people living with diabetes, participated in the project.
