Mark Clements, MD, PhD
Co-Chair
Mark is a Professor of Pediatrics at the University of Missouri-Kansas City and the Rick and Cathy Baier Family Endowed Chair in Endocrinology. Mark is a pediatric endocrinologist and clinical researcher who collaborates broadly across the field of diabetes research. Responsible for setting the overall strategic direction of the RTA.

JUAN ESPINOZA, MD
CO-CHAIR
Dr. Juan Espinoza is the Chief Research Informatics Officer for Lurie Children’s Hospital, and Associate Professor of Pediatrics at the Feinberg School of Medicine at Northwestern University. He will serve as contact PI for the grant, and co-lead on the project along with Dr. Clements at Children’s Mercy Kansas City. Dr. Espinoza will lead the Administrative Core and co-lead the Demonstration Project Core with Dr. Clements. Dr. Espinoza brings years of experience to this project in CGM-EHR Integration, health information systems, advanced analytics, and health equity-focused research.

Grace Garcia, MHA, INF, BSN, RN
Program Manager
Grace Garcia serves as the Program Manager for the Rising T1DE Alliance. The program manager will oversee the day-to-day operations of the entire project, provide supervision to project staff, and coordinate across cores and implementation sites. She will also help fulfill reporting requirements and ensure financial responsibility and oversight of the grant. As a registered nurse with a master's degree in health administration and informatics, Grace brings expertise in clinical quality improvement, implementation science, and product management.

Dominique Pahud, MBA
Strategy and Growth Consultant
Dominique Pahud serves as the Strategy and Growth Consultant for Rising T1DE Alliance. This role works with stakeholders to develop and implement a strategic plan that aligns project goals with organizational priorities, conducts market analysis, and establishes performance metrics to ensure successful outcomes.

Brent Lockee
Technology Lead
Brent Lockee is the lead data scientist and serves as the Technology Lead for the Rising T1DE Alliance. This role is responsible for overseeing the development, validation, and implementation of the Diabetes Data Dock which involves ensuring consistency across data sources, optimizing algorithms for data harmonization, and collaborating with clinical and technical teams to enhance interoperability.

Lawrence Lett, MS
Project Manager
Lawrence Lett serves as the project manager for the Rising T1DE Alliance. This role will be responsible for overseeing the planning, execution, monitoring, and closing of this research project. The primary objective is to ensure the project is completed on time, within scope, and budget while meeting the highest standards of quality and compliance.

Sadaf Javaid, Med
Training & Dissemination Specialist
Sadaf Javaid serves as the Training & Dissemination Specialist for Rising T1DE Alliance. This role is responsible for drafting the components of the CGM-EHR Integration toolkit, and developing supporting materials as needed. This role will create multimedia materials to support training and onboarding for the implementation sites and will help develop and execute on the T1DX QI Clinic Network dissemination plan.

Eric Williams Jr. MSHA, MSHI
Informatics Lead
Eric Williams, Jr. serves as the Informatics Lead for Rising T1DE Alliance. In this role, Eric will provide critical informatics expertise to the project, including interoperability, common data models, data standards and terminologies, and EHR integration. Eric will work closely with the Demonstration Project and Data Analytics Cores, as well as provide technical expertise to each of the implementation sites. This role will also contribute technical writing and expertise to the CGM-EHR Implementation Toolkit development.
Core Teams
Coordination, Research, and Dissemination Core: This team oversees the project's financial and administrative governance, facilitates the assembly of a community advisory board, and collaborates with HCT to meet reporting requirements. It also coordinates with CGM manufacturers, NCQA, and T1DX. Additionally, the team manages research projects, white papers, and other key outputs.Technology, Data and Analytics Core: This team will develop standardized data dashboards and applications for use and implementation across all sites. It will also enhance the D-Data Dock software, incorporating new features based on site-specific needs assessments. Additionally, the team will provide support for each cloud-hosted instance of the D-Data Dock at demonstration sites. Key roles include Clinician Design Champion, Senior Data Scientist, Data Scientist, Data Engineer, Software Project Manager, and Mixed Methods Scientist.
Implementation Core: This team supports implementation sites through CGM integration and D Data Dock deployments, including the development of PDSA cycles. It provides technical guidance, as well as clinical and project management support. Additionally, the team coordinates training, dissemination, and collaboration efforts to ensure effective implementation.
Oversight and Advisory Partners
The Community Stakeholder Advisory Board (CSAB) is chaired by Sanjeev Mehta (Joslin) and provides guidance on the future large-scale implementation of the D-Data Dock and its approach to personalized, data-driven, and iterative care model design. The CSAB is made up of representatives from implementation sites (Lurie Children’s Hospital, University of Cincinnati and Kansas University), the T1D Exchange, the National Committee for Quality Assurance, the Helmsley Charitable Trust, clinicians, researchers, and patient and family advocates.
The Leona M. and Harry B. Helmsley Charitable Trust (Helmsley) is a global philanthropy dedicated to improving lives today and creating healthier futures through strategic investments in areas like Type 1 diabetes, rural healthcare and vulnerable populations. Their support has been vital to the Rising T1DE Alliance, helping to tackle challenges in diabetes care by driving data integration and interoperability efforts.
The National Committee for Quality Assurance (NCQA) is a nonprofit organization dedicated to improving healthcare quality through measurement, transparency, and accountability. As a Rising T1DE Alliance advisory board member, NCQA brings expertise in developing evidence-based standards and performance metrics that guide the delivery of high-quality, patient-centered care. Their insights help ensure that our innovations align with best practices and drive meaningful improvements in outcomes for individuals with Type 1 diabetes.
The T1D Exchange (T1DX) is a nonprofit organization dedicated to improving outcomes for individuals with Type 1 diabetes through research, collaboration, and innovation. By fostering a robust data ecosystem that includes patient-reported outcomes, clinical insights, and real-world evidence, T1DX drives advancements in care and treatment through intentional quality improvement. T1DX and RTA are partnering to develop data tools that support both T1DX members and D-Data Dock users and will eventually help more organizations integrate and use diabetes device data.
