# CGM DATA POLICY

### **OVERVIEW**

Continuous glucose monitors (CGMs) have transformed diabetes care, giving people and their care teams a more detailed view of glucose patterns throughout the day. But the systems around those data have not always kept pace. CGM information often remains in device-specific apps and platforms instead of moving easily into electronic health records or across healthcare settings. That can make it harder for patients to share their data, for clinicians to see the full picture, and for health systems to use CGM information to support more proactive, coordinated care.

 Through the CGM Data Policy Initiative, the Rising T1DE Alliance brought together device manufacturers, data aggregators, patient advocacy organizations, people living with diabetes, caregivers, and quality organizations to examine these challenges and identify practical paths forward. The resulting white paper reflects a year-long cross-sector process informed by three patient and caregiver surveys. The discussions focused on interoperability, data governance, contracting, and common data definitions. Patients consistently emphasized that they want their data to be used to help them while preserving transparency, choice, and appropriate safeguards around how that information is shared and used.

The white paper outlines six recommendations designed to make CGM data more accessible, interoperable, consistent, and useful across the healthcare ecosystem. Together, they address patient access to device data, common interoperability standards, information-sharing protections, baseline platform capabilities, shared contracting principles, and standardized CGM data definitions. The goal is to help create a more connected data ecosystem in which CGM information can support better care while keeping patients at the center of how their data moves and is used.

### CGM DATA DICTIONARY

For CGM data to move reliably between devices, applications, and healthcare systems, the data must have consistent meaning. As part of the **CGM Data Policy Initiative**, RTA developed a shared CGM Data Dictionary that brings together common CGM measures and documents their definitions, calculation methods, terminology, and areas of alignment across participating manufacturers and data platforms. It builds on existing standards including iCoDE, LOINC, and the Ambulatory Glucose Profile and is intended to support more consistent clinical care, analytics, research, and quality measurement.

##### RTA Response to the CMS Health Technology Ecosystem RFI

 In June 2025, the Rising T1DE Alliance responded to the CMS and ONC Request for Information on building a more transparent and interoperable digital health ecosystem. RTA’s comments focused on a major gap in today’s health data infrastructure: medical device data, including CGM and insulin-delivery data, often remains outside the access and interoperability frameworks used for electronic health records. The response outlined approaches to patient access, FHIR-based interoperability, common data standards, population-level data access, extending information-blocking protections to device data portals, and the integration of device data into data-driven and value-based care. ##### RTA Public Comments on the HTI-5 Proposed Rule

In February 2026, RTA submitted public comments to ONC on the HTI-5 Proposed Rule and its continued move toward FHIR-based, API-enabled interoperability. The comments highlighted the importance of maintaining clear baseline expectations as interoperability expands, particularly for auditability and traceability, authentication and authorization, encryption, and API performance and availability. ##### Data-Driven Care & Value-Based Care

Making CGM data available is only useful if healthcare organizations can turn those data into better care. RTA’s work in value-based care explores how integrated CGM and clinical data can support population health management, risk stratification, proactive outreach, and care between visits. ##### RTA Perspective on HTI-5

HTI-5 signals a continued shift toward FHIR-first, API-based interoperability while proposing significant changes to the federal health IT certification framework. RTA’s analysis examines what those changes could mean for healthcare organizations, including the importance of auditability, authentication, API performance, security, and governance as data exchange becomes increasingly automated. ##### iCoDE Standards Framework

The Integration of Continuous Glucose Monitoring Data into the Electronic Health Record ([iCoDE](https://www.diabetestechnology.org/icode/icode2.shtml)) facilitates the efficient upload and integration of continuous glucose monitor (CGM) data into the Electronic Health Record (EHR). The project aims to develop technical integration specifications and data integration facilitation guidelines. Over 130 individuals from more than 60 academic, government and clinical organizations, as well as people living with diabetes, participated in the project.
